This report reveals gaps like inconsistent consent processes, limited IT support, and low interoperability that can hinder collaboration, especially for smaller nonprofits. The CIE Initiative recommends seven actions: standardize legal templates, digitize referrals, secure and connect data systems, track real-time service availability, and expand training and technical support. These strategies create a connected, efficient system that helps residents access services faster and enables providers to coordinate care more effectively.
